Feedback Process

Emergency Health Services (EHS) is an evidence based organization that relies on client and provider feedback to improve quality and enhance efficiencies.

This patient focused system requires a method for clients, providers and system administrators to easily query EHS operations to ensure that it is meeting the clients' needs. As a result, the EHS system has implemented a feedback process that provides anyone, who has come in contact with the system, an opportunity to inquire about processes, protocols, response times, paramedic interaction, or any other issues that may be unclear or unsatisfactory to the client. This process is designed to be client friendly and provide prompt and appropriate responses. All inquiries received by EHS, are viewed as an opportunity to validate the system, and enhance client satisfaction, as well as to drive system improvements and efficiencies. In this feedback process, all inquiries are logged and categorized in order to establish trends in issues identified so the system can use this information to improve upon itself.

The EHS Feedback form is to be used by anyone wishing to discuss/inquire about the service provided by EHS and/or an EHS Contractor. Please submit your comments, questions or concerns to EHS (Attn: EHS Feedback Coordinator) by using the form and mail or fax it to the address/fax number located on the top left hand side of the form. After receipt of your inquiry we will assign it an ID number and designate it for investigation and follow-up. This process enables us to track your inquiry and ensures a rapid response.
